Manuel Akanji was standing in an offside position, directly in front of Bernd Leno when Nathan Ake sent a downward header straight at the pair of them. Akanjki jumped in the air, parting his legs so the ball could travel through them and creep into the corner. It looks an astonishingly bad decision, unless Akanji was somehow onside.
